ACCIDENT WAIVER AND RELEASE OF LIABILITY

By signing this Waiver, I assume all risk of my child and/or for myself participating in aviation activities, including flight. Without signing this form, neither myself or my child will be able to participate in any activity sponsored by Aviation Academy of Louisiana, LLC. I acknowledge that the above activity may pose some risk of personal injury and that I undertake and assume this risk for myself and my child.

On behalf of myself and/or my child, I further waive, release and hold harmless Aviation Academy of Louisiana, LLC, Craig Simon, Robinson Helicopter Company, the pilot flying the aircraft, and the participating school from any and all liability.

I assume all risks on behalf of my child as a result of my child's participation in the above activity.

I certify that I have read this document in its entirety and fully understand the contents. I understand this is release of liability and indemnification of liability for myself and my child, and I sign it by own free will.
